We’ll send over a preliminary questionnaire, and then arrange a convenient time for your in-home assessment to discuss your/your family member specific needs. During this complimentary initial assessment, one of our experienced registered nurses will work closely with you to create a personalized care plan tailored to your/your family member unique requirements.

Nope! No upfront payment is required for our services. If you or your loved one is under the age of 21 and eligible for the GAPP program, all costs are fully covered through Medicaid, and no payment will ever be due. For those seeking private care, payment is only required after the initial assessment has been completed and a caregiver has been matched to meet your needs.

What sets us apart from other home health agencies is our flexible approach to care. We have a one-hour minimum per visit, ensuring accessibility and convenience. If needed, services can be extended to full-time care, available 24 hours a day, 7 days a week. The final decision regarding the number of approved service hours is determined by the Department of Community Health’s medical review team. The hours permitted will be based on your child’s specific medical needs and requirements for activities of daily living (ADLs).

All service applications are carefully evaluated by the Department of Community Health’s medical review team. After Salvation Private Home Care completes the client intake process, the collected information is submitted for review to determine approval or denial of services.
With our extensive experience in this process, we proudly maintain a 97% approval rate for applicants pre-qualified during our initial intake assessment . However, while we strive to provide the best guidance and support, Salvation Private Home Care cannot guarantee approval of services.

No, Salvation Private Home Care does not require a long-term contract. Upon starting services, you’ll sign a Service Agreement outlining our commitment to deliver the care you need during the days and times you select.
We understand that needs can evolve, which is why our nurse is always available to reassess your situation and recommend adjustments to your care plan. You also have the flexibility to modify or discontinue services at any time, without penalties.
